1.1

The pharmacy's standard operating procedures have not been suitably reviewed. And the team members have not read and signed these. So, they may be unclear about the safest and most effective ways to carry out tasks.

All sop’s have now been revised, amended where necessary signed and properly dated

Each employee will be issued with their own file with a set of SOP’s customised to match their respective competencies/responsibilities. These will form an annex to a portfolio of documents consisting of
1. Contract of employment
2. Training record
3. Appraisal
4. Staff handbook
5. Error recording (Team discussion of all errors carried out monthly).
All staff will be required to read and declare that they have read and understood the contents of this file.

The staff handbook covers a large range of topics including safeguarding, whistleblowing, confidentiality, IG and health & safety.

The pharmacy team does not record mistakes in a regular manner. And it has limited arrangements in place to learn from mistakes. So, the team may miss opportunities to improve and make services safer.

Errors to be recorded in each staff members file. Group discussions of errors to be conducted monthly in confidence.

2.2

All staff must be qualified for their roles and undertake formal training in accordance with the General Pharmaceutical Council’s minimum training requirements.

Formal training via buttercups to match the roles of staff has been applied for and should commence in the New Year.

3.1

There are risks of trip hazards within the premises. These may be detrimental to people using the consultation room or when drivers make deliveries.

Surface faults to carpets have been temporarily repaired to make safe.

Dog and his toys removed from premises.

4.3

Arrangements to ensure stock remains fit for purpose are insufficient. The pharmacy has not recorded fridge temperatures for a period of time therefore there is some risk that it is not storing fridge-line medicines at appropriate temperatures, rendering these items unfit for purpose.

A new easy to operated Fridge thermometer has been introduced with a nominated person and deputy to manage readings.
Readings recorded on computer fridge monitoring template as was previously instructed.
Fridge alarm system is functional.
