Although the pharmacy's prescribing service appears to be low volume, the pharmacy does not record consultation notes for the prescriptions issued. In addition, it does not always make a record about the prescriber for the private prescriptions it dispenses. Its responsible pharmacist record is not always completed. And it does not record all the information it needs to about medicines it supplies against Patient Group Directions (PGDs). For example, some of its weight loss treatment forms did not always record details about weight when a repeat supply was made. And some answers on the forms were unclear, for example questions were sometimes just answered with a tick. Some vaccination forms were missing a record about any advice given.

The Pharmacist is now recording all consultation notes for the private prescriptions supplied and dispensed and identifies themselves as the responsible Pharmacist and prescriber. They are following all the relevant PGD’s required and making all this evidence clear for inspection.
All necessary records are kept in files or digitally
