The pharmacy does not do enough to ensure that it stores all its medicines properly.

Split Pack Dispensing
Retrain all team members on the correct procedures to follow when dispensing a split pack of medicines.
Any split pack must only contain that drug with no other mixed batches or brands.
All split packs must be clearly marked with a line on each face of the box.
When dispensing from a split pack, ensure the BN of medication and box match or check with RP if unsure.
Signed training guide attached.

Expiry Date Checking

A Full date check of all the Pharmacy stock has commenced. A new Date check matrix has been created sectioning the pharmacy into clear categories. It also identifies the date of checking and who is checking the stock. Any stock with less than 3 months expiry are removed from the dispensary shelves and kept on a separate short, dated stock shelf at the rear of the pharmacy.
Signed training guide attached and new date checking SOP attached to reflect new changes.

Fridge Temperature

All team members have been re-trained on accurately reading and re-setting the temperature on the electronic fridge.
A guide for this has been printed and attached to the front of the fridge.
Regular temperature readings are being recorded daily and a Note to this effect has also been attached to the front of the fridge.

Methadone storage

All Pharmacists have been advised that Methadone must always be stored in the CD cabinet. A written notice is also attached to the front of the CD cabinet.
