1.1

The pharmacy team do not keep regular records of near miss mistakes. This means that they are not able to demonstrate learning from previous incidents.

• Ensure all pharmacy colleagues can log into the internal reporting system, QI and understand how to report a near miss and Patient Safety incident on QI tech.

• Arrange a team huddle to set expectation with the whole team around the reporting of any near misses and any patient safety incidents, which must be completed in a timely manner.

• Review all near misses and PSIs recorded on QI tech once a month.


• Record any learning identified from near misses and PSI on the monthly patient safety report.


• Near Miss Compliance sent to store twice per week. Monday and Thursday, to ensure compliance is met by the pharmacy team.

• To ensure the monthly Patient Safety Report is completed.

• Utilising QI Tech to assist the AOM to check compliance on Near Misses and monthly Patient Safety Report.

3.1

The pharmacy team were storing stock in such a way that increased trip hazards to staff.

• Order additional handheld devices in the pharmacy to allow pharmacy team to scan and place the prescription bags from the Central Fulfilment (CF) Hub to the retrieval area.

• Place all prescriptions bags returned from the CF Hub in the delivery totes to the retrieval area.

• Create additional space in the retrieval area.

• Ensure pharmacy team are aware of Business Continuity Plan in scenarios that involve an IT system failure.

• Remove all empty delivery totes back to the wholesaler.

• Deep clean the pharmacy.

The pharmacy team did not store all medicines in containers which complied with legal requirements.

• Remove and dispose all medication stored in white boxes.

• Ensure any uncollected medication which is returned to pharmacy stock is either disposed of or correctly labelled with batch and expiry.

The pharmacy teams stored medicines in such a way that may increase the risk of selection errors.

• Ensure all medication packages returned from the CF hub are placed immediately in the retrieval area according to the internal Best In Class process.

• Clean and tidy the dispensary shelves
