The pharmacy does not identify and manage risk appropriately. The pharmacy team do not routinely record near miss mistakes.

Every single near miss mistake is being strictly recorded since our inspection. This will be discusses and reviewed during our monthly team review meeting

The pharmacy team could not show that they safeguard vulnerable adults and children adequately.

There is a poster in a pharmacy by the printer showing the phone numbers to be used to report or find advice regarding safeguarding issues related to both adults and children.
A comprehensive risk management action plan can be found on our google drive to which all my colleagues are having unrestricted access to. I did make sure that they are aware now of all the above.

The pharmacy team do not keep and adequately maintain all of the records necessary for the safe provision of pharmacy services.

The CD balance is being checked every two weeks instead of monthly.
PILs will be made available on regular basis to patients.

A folding lockable door will be installed to protect the storage of multi-compartment compliance aids. In principle agreement has been reached with the landlord and we are awaiting for the final confirmation.
