Staff will be trained on the new procedure to ensure consistency and compliance. |

| 4.2 | The pharmacy's dispensing process currently does not empower the pharmacy team members to make meaningful clinical interventions on most of the prescriptions it dispenses. It largely relies on the prescribing service to make clinical checks about medicines people are prescribed. This increases the risk that people are supplied with medicines that are not clinically appropriate for them. In addition, the pharmacy provides services to a third party pharmacy with limited assurances that the medicines are being appropriately dispensed to patients. |
● During the initial inspection it was pointed out that not all pharmacist colleagues had correct access to patient records. Subsequently all pharmacist colleagues including locums have access to full medical history on patient records. This includes past medical history, medicines prescribed, previous consultations and are empowered to make meaningful intervention with the prescribing team. ● The third party pharmacy has been given notice with our last contracted day being the end of 28/10/25.
