The pharmacy doesn't have standard operating procedures or prescribing policies for its private prescribing service available for team members to refer to. This means that the pharmacy cannot show how it ensures team members, including the prescriber, work consistently and provide the pharmacy’s services safely.

To address this, the pharmacy is undertaking a full review and consolidation of its SOPs and prescribing policy framework. All relevant SOPs and policies will be formally documented, reviewed, and signed off. Approved SOPs will be centrally stored and made readily accessible to all team members. Associated staff training will include documented SOP acknowledgement and records to ensure consistent understanding and application of procedures.

The pharmacy doesn’t appropriately verify the information people provide when requesting weight loss treatment. The prescriber uses photos to assess people’s BMI without any further independent verification of the information people declare relating to their weight, height and health conditions on the assessment form. So there is a risk that people may get medication that is not appropriate for them.

As part of our ongoing commitment to safe and responsible remote prescribing, the pharmacy will be implementing enhanced patient assessment and verification safeguards. Safeguards being introduced include: • Contemporaneous photographic verification for BMI assessments – Patients will be required to submit live photographs captured in real time through the platform. The ability to upload previously taken or potentially edited images will not be possible. • Independent verification of patient-submitted weight – Patients will be required to provide a live photograph showing their feet positioned on weighing scales with the weight reading clearly visible. • Escalation of discrepancies or clinical concerns. Any inconsistencies, anomalies, or clinical concerns identified during assessment may result in a request for a video consultation prior to prescribing. These safeguards are intended to strengthen clinical safety, support appropriate prescribing, and ensure continued regulatory compliance.
