Prescribers do not regularly record consultation notes when prescribing and do not consistently demonstrate how they make their decision to prescribe or not. And prescriber records do not contain the level of detail that includes information such as treatment goals, ongoing monitoring or consideration for any follow-up required. This makes it harder for the pharmacy to demonstrate continuity of care and show the supplies it makes are appropriate.

Prescribing decisions are currently made in line with established clinical protocols and structured assessments, which capture the core information required to determine suitability for treatment within our defined, condition-specific service. Prescriber, pharmacist and dietitian notes at point of initial assessment were already available.

To strengthen visibility across the prescriber team, prescribers have been asked to more consistently record their clinical rationale to support continuity, with emphasis on exceptions, while remaining proportionate to the scope of the service.

The pharmacy does not always use appropriate safeguards when prescribing weight-loss medicines. Prescribers regularly use real-time photos for initiating treatment without any further independent verification of the information people declare relating to their weight and height on the assessment form. And they do not check that real-time selfies match the photos provided for identification checks. Pharmacy team members carry out video consultations only for certain groups of people. And prescribers cannot demonstrate how they record people’s specific circumstances when they choose not to use two-way communication to support their prescribing decision.

Identity verification currently uses a layered, risk-based approach with real-time photographs used alongside automated background verification and consistency checks against declared patient details, including age and sex.
Identity checks will now also be included as part of our expanded video consultations.

All controls are reviewed through ongoing audit and governance processes to ensure they remain proportionate, effective and responsive to identified risks. These measures will be kept under regular review and adapted in line with emerging regulatory expectations and sector guidance.
