Risk reviews to be completed for services offered and then to improve services as identified. Near miss recording and learning to be a primary part of the dispensing process and all pharmacists will have their own logs. A pharmacist lead will analyse reflect and review all recordings and will advise of any changes needed. Logs will be held using the PMR tool and RPS template recording sheet. For the staff we will also encourage a reporting process through the PMR system and training will be given on how to use the near miss and incident feature. |
