The risks associated with the hub and spoke model for dispensing compliance packs are not sufficiently identified and managed. The written procedures do not cover all aspects of the hub and spoke arrangement or outline who is accountable at each stage of the process. This is made more complicated due to the company structure.

‘Nomad factory’ is the term used in this document for the Brierley Hill branch.

The accountabilities of each pharmacy that uses the Nomad Factory will be decided so there are clear, and documented lines of accountability and responsibility. This will be shared with the pharmacy teams that use the Nomad Factory. This should also be agreed with each of the Superintendents that have pharmacies that transfer prescriptions to the Nomad Factory.

The risks associated with using the Nomad Factory to be identified and actions identified to reduce the risk;
Such as –
MDS transfer process and gaining patient consent.
Staffing levels, capability and training.
Business continuity (include some of the ‘worst case scenarios’ such as power failure at Brierley Hill, pharmacy closure or high levels of sickness)
Delivery procedures
Procedures for CD’s, creams and externals and what happens when a separate legal entity is being supplied.

The SOPs for MDS dispensing, MDS transfer and near miss recording to be reviewed and shared with the pharmacy teams.

An ongoing audit, as detailed in the GPhC guidance on providing pharmacy services at a distance will be designed and used to review the ongoing risks. This should be linked to the risk assessment.

Pharmacy teams that use the Nomad Factory will be briefed to ensure that they are aware of any new processes and understand their accountabilities in the process.

Patients to be informed that their prescription will be dispensed at a different pharmacy and by a different company (when relevant).

Consent to be obtained for each patient when they are transferred. The SOP for MDS transfer will be followed, which includes how consent is obtained and recorded.
