A full check of all CD stock has been completed, and the CD registers have been checked against the physical stock. All identified discrepancies are currently being reported to the CDAO. An investigation has been completed into the discrepancies, and the pharmacy team has been reminded of the importance of keeping accurate CD records and following “SOP 002 Controlled Drugs” at all times. All relevant staff will re-read and sign the CD SOP to confirm their understanding. Due to issues identified with the electronic CD register, the pharmacy will return to using a paper CD register only to ensure CD records are completed accurately and consistently. The Area Manager, Sam Adshead, will visit the pharmacy within two weeks to provide additional training to all relevant staff on the correct management of CDs, record keeping and legal requirements. During the visit, Sam will also review the CD registers and records to ensure they are accurate, compliant and being maintained correctly. Weekly CD balance checks will continue, with all checks recorded. The Responsible Pharmacist will review the CD registers and balance checks each week to ensure records remain accurate and any future discrepancies are identified, investigated and reported immediately. |
