The pharmacy is offering a private travel vaccination service via PGDs but the PGDs are not available or accessible in the pharmacy. And so it cannot show that it is using the PGDs appropriately when providing the service.

The pharmacy has printed and electronically stored the Up-to-date PGDs. Signed authorisation records for all practitioners operating under each PGD are available. All relevant staff know where the PGDs are stored and how to access them immediately during service delivery. It has updated its Travel Folder to store patient records efficiently.

The pharmacy cannot show that it always stores its medicines requiring cold storage at the required temperatures. This could mean that people receive medicines that are not suitable for use.

The pharmacy will:
-complete temperature logs electronically, recording the minimum and maximum temperatures of its fridges.
- Complete Refresher training where the objectives include: reinforcing the understanding of cold-chain integrity, ensuring staff can correctly read and record fridge temperatures, clarify procedures for responding to temperature excursions, document issues such as an internal incident or near-miss, identify root causes and record corrective actions taken and review relevant SOPs to reflect monitoring, recording, and escalation processes.
-Ensure all staff have read and understood the SOPs. The training signed template to be kept in clinical governance folder.
