- A lock will be installed on the consultation room door which will prevent unauthorised access while not in use.

- A temperature log has been created on the PMR for the second fridge recently purchased to enable temperatures to be logged daily. - Review the SOP for ‘Recording of fridge temperatures and maintaining cold chain’ and make any necessary amendments. - Ask all staff to read and sign the reviewed SOP and test understanding by asking staff to demonstrate how to record fridge temperatures. - Undertake periodic (approximately every 6 months) audits of the fridge temperature logs and implement any required action.

- Immediate suspension of private prescribing service. - The service will not be re-implemented until a comprehensive review of guidance and resources for providing independent prescribing services has been undertaken. Prescribing policies will be reviewed, amended and put in place before consideration of re-implementation of the service.

It omits the supplier’s address in its controlled drugs register, its emergency supply records are routinely incomplete, and prescriber details are frequently missing or recorded incorrectly in the private prescription register.

- Review the SOP for ‘Recording entries in the CD register’. - Ask all staff to read and sign the reviewed SOP. - Test staff understanding by asking questions to ensure they are aware of the relevant information required when making entries in the CD register – particularly concerning supplier addresses. - Periodically (approximately every 6 months) undertake an audit of information recorded in CD register entries and implement any required action as a result. - Review the SOP for ‘Dispensing of medicines’. - Make necessary amendments focusing on the need for complete and accurate records of prescriber details and correct form types being recorded during the assembly process. Information recorded on the PMR during the dispensing process translates into the information recorded in the private prescription register. - Ask all staff to read and sign the reviewed and amended SOP. - Test staff understanding of the information that must be recorded and accurate particularly in relation to private prescriptions and emergency supplies - Undertake period (approximately every 6 months) audits of the private prescription register looking at complete and accurate information being recorded and implement any actions required resulting from the audit.
