The pharmacy cannot show that all its medicines it needs to keep in a refrigerator have been stored at the right temperature.

Daily Temperature Monitoring:
• Record refrigerator minimum, maximum, and current temperatures daily - all staff members have revisited the appropriate Standard Operating Procedure (SOP).
• Use a calibrated thermometer with digital display and memory function.

Record-Keeping:
• Maintain a temperature log sheet - a paper version is now displayed on the fridge to document readings consistently.
• Ensure records are signed/initialled and dated by the responsible team member.

Escalation Procedure:
• Appropriate SOP has been reviewed by all members of the team for steps to take if fridge temperatures go outside 2–8°C.
• Including immediate actions e.g., quarantine stock, inform RP, check power supply.

Training & Awareness:
• Provide training to all staff on the importance of cold chain storage and how to monitor/record correctly.
• Document training completion in staff records.

Audit & Review:
• Conduct a weekly check by the Responsible Pharmacist (RP) and Cluster Lead to ensure records are complete and accurate.
• Review logs monthly to identify patterns or recurring issues.

Equipment Maintenance:
• A new larger fridge is in place.
