Cambridge Avenue Pharmacy (1032505) - Improvement action plan

Standard not met Reason Action being taken by the Pharmacy By when Notification By Pharmacy Improvements Made

The pharmacy has reduced its staffing levels in recent months. And there is evidence this has led to some key tasks relating to safety and medicines management being missed. This includes recording of near miss errors, near miss error reviews and acting on medicine recalls.

1. Near Miss details to be recorded in full at the time of each occurrence.
2. Near Miss to be reviewed every month in line with LloydsPharmacy expected procedures and documentation.
3. Safer Care to be ‘relaunched’ and completed weekly in line with company scheduling.
4. Drug recalls to be printed daily and actioned. RP to sign the document and file once complete.

Area Support Coach to work at the pharmacy one day/week.
Relief Dispenser to support the pharmacy three days/week.
Cluster Manager to visit the pharmacy twice a week.
Professional Support Manager spending Monday 3rd February at the pharmacy all day to support with the action plan, and will then visit on ad-hoc basis depending on any needs identified.

The pharmacy obtains its medicines from reputable sources. But it does not take timely action to act upon medicine recalls issued by the Medicine & Healthcare products Regulatory Agency as required. There is evidence of recalled medicines on the shelves available for sale and supply.

1. Drug recalls to be printed daily and actioned. RP to sign the document and file once complete.
2. Review of all recalls from the previous 6 months and actions taken as appropriate to remove any affected stock from the pharmacy stockholding.
