The pharmacy does not regularly record near misses and therefore misses opportunities to learn from trends in mistakes.

The regular pharmacist, along with all staff members, has been informed about the importance of maintaining an error log and the value of learning from mistakes. All staff have been strictly instructed to record all dispensing errors without fail. The manager is monitoring the error log closely and has been instructed to report it weekly to the superintendent pharmacist to ensure that all dispensing errors are properly recorded.

Fridge temperatures are not recorded by the pharmacy. This means that the team cannot be sure that medicines stored in the fridge are safe to use.

Staff members were checking the fridge temperature daily; however, they were not recording the readings. The importance of maintaining accurate temperature records was explained to all staff. All staff members have been instructed to record the fridge temperature every morning without fail. The pharmacy has also implemented the “Pharmsmart system” to maintain electronic records of fridge temperatures moving forward.
