The pharmacy cannot always demonstrate that it asks people the necessary questions before administering a vaccination. And it does not always record the date when it records people’s weights for its weight loss service. In addition, it does not always conduct regular audits of certain medicines in line with its standard operating procedures. It does not always make a record about the prescriber for the private prescriptions it dispenses. Its responsible pharmacist record is not always accurate. And it cannot demonstrate that it records all the necessary information when it dispenses unlicensed medicines.

• Full vaccination history taken, patient made aware of ingredients for any possible allergies, check medicine history and any treatment patient may be taking. Template to be competed with date of vaccination, batch no, expiry date etc.
• Full weight history (Weight,BMI) will be recorded with corresponding dates to give patient full weight loss history & to meet eligibility for the supply. This to be recorded in Both PMR & assessment form
• Any medicines ( separated from original container) without batch number and expiry date to be disposed of. Staff must write Exp date & batch no when dispensing from original container.
• All private prescriptions dispensed will be recorded in Private Prescription Electronic Register including details of the prescriber. Reference number to be written on the paper prescription.
• RP will login first thing via the PMR at the start of the shift; any locum pharmacist will also be notified of this. Monthly audit to be done.


• Appropriately complete ‘Certificate of Compliance for unlicensed medicines ‘during dispensing of the medicine & to be kept the actual form in the Special Folder.
