The pharmacy does not maintain clear consultation records for its prescribing service. The prescriber does not consistently document the information discussed during consultations, including the rationale for prescribing decisions. This limits the ability to demonstrate that appropriate checks have been carried out to ensure people understand how to use their medicines safely and know what follow up is required.

Prescriber will take additional training to help support with consultation record keeping and ensuring consultation notes are adequately recorded. The consultation records should reflect consultation method, additional discussion had with patient and rationale for prescribing; and if needed rationale for refusing. There will be up to date record kept of information recorded from patient and relevant information noted from National Summary Care Record with recorded dates of past medical history and verified weight within the last 3 months.

Furthermore, the prescriber will undertake further CPD training to improve their practice in improving consultation record and skills via CPPE.
